Applying to change a pharmacy business licence

The Pharmacy Business Ownership Act 2024 (PBO Act) provides for licence holder/s to apply in the approved form and with the prescribed fee to make the following changes:

  • adding a licence holder - Form 2
  • removing a licence holder – Form 3
  • changing the licensed premises – Form 4
  • adding or removing a material interest holder or director – Form 5, or
  • other changes (e.g. changing a business name) – Form 6.

To make changes to your pharmacy business licence, please follow these steps.

  1. Download the relevant form and annexures.
  2. Review the guidance material available on this website for information on how to complete the forms.
  3. Prepare application by completing relevant forms, annexures and gathering supporting documents
  4. Register to securely upload the application via Kiteworks.


  1. Use the Kiteworks link received by email to upload application securely.
  2. Pay the application fee advised by the Council via BPOINT and keep a copy of the receipt.
  3. Respond to requests from the Council and await decision.

Notification obligations of licence holders and material interest holders

Notification of change relating to holder of material interest in pharmacy business or director of licence holder

If a licence holder becomes aware of the following, they must notify the Council within 14 days using Form 7:

  • a change in the number or type of shares a person holds in the holder; or
  • a change in a person’s material interest in the pharmacy business to which the licence relates, including, for example, a person acquiring a new material interest in the pharmacy business (like a child of a practising pharmacist becoming 18 years of age and joining a class of beneficiaries etc.) or disposing of the person’s material interest in the pharmacy business; or
  • a change of a director of a holder of the licence.

Other notifications

Licence holders, or directors or shareholders of a corporate licence holder, are obliged to notify the Council of the following:

  • a conviction of the licence holder, or a director or shareholder of the licence holder of an indictable offence – within 14 days after conviction
  • any changes in the matters listed in section 72(a) to (e) or (g) to (j) of the PBO Act related to fit and proper person criteria – within 14 days after change
  • the death of a joint licence holder – within 14 days after death
  • a change in contact details of the licensed pharmacy business – within 14 days after change
  • proposed temporary closure of the licensed pharmacy business – prior to proposed closure (unless the licence holder has a reasonable excuse e.g. due to an unexpected disaster event such as a cyclone or flood)

Please email notifications to enquiries@pboc.qld.gov.au. The Council will confirm receipt of the notification.

Other matters

For any of the following matters, contact the Council for further advice:

  • A licence holder dies (and the licence is not held jointly).
  • A licence holder, or a director or shareholder of a corporate owner, has their general registration cancelled under the Health Practitioner Regulation National Law.
  • A director, shareholder or beneficiary is no longer the spouse of the practising pharmacist in the pharmacy business.

PBS approved suppliers

If you are approved to supply Pharmaceutical Benefit Scheme (PBS) medicines, you should consider whether you also need to notify the Commonwealth Department of Health, Disability and Ageing about any changes to your business.
